Appoinments

We see patients during regular office hours by scheduled appointment only. You can download and fill in this Patient Information Record before your first visit.

When you call to make an appointment, please ask for the appointment desk. The appointment clerk will make every effort to keep your holding time to a minimum. Please try to understand that certain times of the day are busier than others. (Before 9:30 a.m. and after 3:30 p.m. are usually quiet times). If you are calling long distance, please tell us so we may expedite your call. The appointment clerk will need to ask you a few questions about your condition. These questions let her determine the urgency of your appointment request. In some difficult cases, when an appointment is not immediately available, she may take a message for the doctor, so that he or one of his assistants may return your call. To schedule an appointment while in the office, go to the reception area (where you checked in for your appointment). If you were seen today, the receptionist will need to see your copy of the "Attending Physician's Statement". If your insurance plan requires an authorization from your Primary Care Physician (PCP), we will need to have a current copy of your authorization in our office before we can schedule your appointment. Your PCP may fax an authorization to us at 918-745-2141. Please allow 24 hours for us to process your authorization

If you want another member of your family to be seen at the time of your appointment, arrangements should be made ahead of time. We may have to reschedule your appointment so you may both be seen at a time when two consecutive appointments are available. As in any medical practice, we periodically treat patients with emergent medical problems requiring immediate evaluation and treatment. Due to these and other unforeseen problems, our doctors may run behind schedule, causing delays in your scheduled appointment. We assure you that all efforts will be made to see you as close as possible to your scheduled appointment time. We apologize for any inconvenience you might face.

We schedule appointments up to 6 months in advance. As a courtesy we try to contact you by telephone two days before your appointment to remind you.

If you need to cancel an appointment, please let us know 24 to 48 hours in advance, so another patient may be seen in your place. (A charge will be made for an appointment which is not kept and not cancelled 24 hours in advance.)

If your doctor wants to see you again over six months from now, we have a reminder system. We will mail you a postcard before you should be seen again to remind you to call the office and schedule an appointment. Call the office when you receive the reminder.
